Overall Objective of the Project: 

The project’s overall objective is to improve the capacity for institutional cooperation on the part of public authorities and key social and healthcare service operators (public and private) dedicated to caring for the Program area’s elderly, in order to come up with joint solutions for the common challenge of ageing, using an economically competitive cross-border model for the integrated management of healthcare for the elderly, which will become a key element in terms of social cohesion.

Project summary: 

The CrossCare project tackles the Program area’s common challenges relating to its ageing population, with all of the relative economic and financial implications, the imbalances in the services relating to social inclusion, and the welfare systems in the cross-border area. The project’s overall objective is to improve the capacity for institutional cooperation on the part of public authorities and key social and healthcare service operators (both public and private) dedicated to caring for the Program area’s elderly, in order to come up with joint solutions for the common challenge of ageing. This objective will be pursued through a cross-border Memorandum of Understanding signed by the competent territorial public authorities, which will establish a joint model for the integrated and personalised care of the elderly. The main outputs are the following: a Cross-border Memorandum of Understanding, a Joint cross-border model for the integrated and personalised management of healthcare for the elderly, and specific joint training courses for social and healthcare workers. The beneficiaries will consist of the national, regional and local public authorities, social and healthcare workers, and, above all, the elderly, who will be able to obtain personalised assistance with a constant improvement of their well-being, quality of life, and social inclusion. By improving the relationships between the public authorities in the social services and healthcare sectors, and through the exchange of best practices between key operators, the cross-border approach will provide for a harmonisation of the policies, and will increase the knowledge necessary for the co-design and implementation of an effective, efficient, and integrated model. This innovative approach to the integrated and personalised management of the elderly and their family members will focus upon the needs of the individual, and will supersede the current systems, which only provide for standardized services.

Main results: 

The CrossCare project will increase the intensity of the cross-border cooperation within the Program area between the citizens and the public authorities responsible for providing social and healthcare services to the elderly. The project’s primary result consists of an increased capacity for cross-border cooperation among the public authorities in the social services and healthcare sectors and the stakeholders responsible for providing care services for the elderly. This result will be pursued by strengthening the cross-border institutional relationships that will be guaranteed by the partnership itself and by the associated partners, as the project involves the collaboration of various social services/healthcare policy makers in the Program area, who will jointly prepare a cross-border memorandum of understanding that will help strengthen the institutional and administrative capacity for the implementation of social and healthcare services. New channels of communication and lasting collaboration will also be created between the project partners, who represent key stakeholders responsible for caring for the elderly in cross-border area. The project’s secondary result consists of an increased capacity for social services and healthcare governance on the part the public authorities in the social services and healthcare sectors and the relative stakeholders, through the establishment of a unified, integrated cross-border model of accompaniment for the elderly, as well as through the cross-border harmonisation of vocational training systems for social services and healthcare workers, for the new care manager figures, and for the families of citizens. The model is based on the concept of active ageing and a welfare community methodology, which, on the one hand, will guarantee economic sustainability, and, on the other hand, will provide for the creation of an Italian/Slovenian strategy for the integrated harmonisation of the governance of healthcare for the elderly.

Objectives and Activities

OBIETTIVI PRINCIPALI DEL PROGETTO

CrossCare integra la cooperazione istituzionale dei servizi territoriali dedicati alla cura dell’anziano al fine di pianificare soluzioni congiunte per la sfida comune dell'invecchiamento, attraverso un modello transfrontaliero per l’assistenza all’anziano. Il modello identifica la nuova figura professionale del “care manager”, prevede la costituzione di Punti Servizi Anziani all’interno delle Case di Riposo e qualifica il servizio domiciliare come elemento chiave nelle strategie di invecchiamento attivo.

 

SITUAZIONE ATTUALE DELL’IMPLEMENTAZIONE PROGETTUALE (30/05/2018)

Le attività iniziali si sono concentrate sull’organizzazione del kick-off meeting, dei comitati scientifici e riunioni ad hoc sul tema ICT. È stata elaborata una prima bozza del Protocollo Transfrontaliero e si è proceduti all’allestimento dei PSA presso le Case di Riposo (CdR). Sono stati avviati i corsi Gentlecare e Validation per gli operatori delle CdR e la formazione dedicata alla nuova figura professionale del “care manager”.
